All new patients must be by referral only. Fax referrals to (704) 235-1971.

In order to prevent delays at the time of your appointment, please have the following ready at the time of your appointment:

• New patients; bring a completed New Patient Registration Form. Download New Patient Form
  Returning patients; bring a completed Return Patient History Form. Download Return Patient Form

• All of the medications you are currently taking.

• Your insurance cards.

• Any medical records you may have pertaining to your problem.
• Please be prepared to pay any co-payment due at the time of the appointment. Mastercard, Visa and personal checks are accepted.

If you need to cancel or reschedule your appointment, please notify our office at least 24 hours in advance.
